Registration Information and Form
Registration
Participation in the IMIG 8th International Conference is limited to registered participants. The registration
fee is $350 ($400 after September 1, 2006). Students, Post-docs, Residents, and Fellows are eligible to
register at a discounted rate of $100, with a letter of verification from the Program Director.
The registration fee includes the following:
- Conference attendance
- Issue of the journal Lung Cancer, containing conference abstracts
- Continental breakfast for all four days
- Lunch for the first three conference days
- Welcome dinner on Wednesday, October 18, at The Signature Room on the 95th floor of the John Hancock Center (http://signatureroom.com)
- Reception on Friday, October 20, at the Riverside Café - located in the Sheraton Chicago Hotel
Optional Social Events:
- Thursday, October 19: Reception and dinner at the Field Museum of Natural History featuring a guided tour of the exhibit "King Tutankhamen and the Golden Age of Pharaohs". There is an additional $50 per person charge for this activity. (http://www.fieldmuseum.org/tut)
- Saturday, October 21: Gala closing dinner at the International Museum of Surgical Science. There is an additional $50 per person charge for this activity. (http://www.imss.org)

Telephone registrations cannot be accepted.
After October 13, please register onsite.
As space is limited, early registration is suggested. Notification of cancellation must be submitted in writing. A $100 cancellation fee will apply to cancellations received before September 15, 2006. No fees will be refunded for cancellations received after September 15, 2006.

Airport Shuttle
Continental Airport Express provides shuttle service to and from both Chicago airports to the Sheraton Chicago Hotel.

Public Transportation
The Chicago Transit Authority's (CTA) Blue Line provides 24-hour access between downtown Chicago and O'Hare International
Airport. Trains run approximately every 8 minutes from 5 a.m. to 11 p.m. Monday through Friday and approximately every
ten minutes from 5 a.m. to 11 p.m. on weekends. Currently, a one-way fare for the 45-minute ride is $2.00.
The CTA station is located at the lowest level of the Main Parking Garage (next to elevator center 4) and is accessible by pedestrian tunnels located in Terminals 1, 2, and 3. From Terminal 5, take the Airport Transit System (ATS) to Terminal 3 and follow the signs.
